Delta Care Connect is a local team that helps people in rural Louisiana stay connected to their doctors and feel better at home. When your doctor, clinic, hospital, or health plan refers you to us, a trained person from your community comes to your house — to listen, help with simple health tools, and connect you to rides, food, and medications when you need them.
There is no cost to you for our coordination services. Your care is referred to us by your doctor, clinic, hospital, or health plan.
What we help with
Help with the everyday parts of staying healthy.
Easy-to-use health devices
We set up simple tools like a blood-pressure cuff or a glucose meter and make sure the first reading works before we leave.
Regular check-ins
We stay in touch between doctor visits — by phone, text, or in person — to see how you're doing.
Help with rides
We help you get to appointments and the pharmacy when transportation is a barrier.
Help with food
We connect you to local food resources when getting healthy food is hard.
Help with medications
We help make sure you understand your medications and can afford to fill them.
What a home visit looks like
A friendly visit — on your schedule, in your home.
You don't have to drive anywhere. You don't have to fill out a lot of paperwork. We come to you, take time to listen, and help with the things that get in the way of feeling your best.
Visits usually take about an hour.

We call or text first to set up a time that works for you.

A friendly Community Health Worker comes to your front door — wearing a name badge — and shows ID.

They sit down with you, listen, and learn what's going on day-to-day.

They help set up your health device and walk you through how to use it.

Before they leave, they make sure your first reading worked and answer your questions.

They stay in touch and share important updates with your doctor or care team.
