Delta Care Connect employs and transports trained Community Health Workers who deliver in-home remote patient monitoring, care coordination, and social-needs support — keeping high-risk rural patients connected to their providers between visits.

We are not a hospital, a traditional clinic, or a software-only platform. We are field-based rural health infrastructure — trusted local people, paired with connected technology, working alongside licensed providers.
The problem we solve
They are logistical, technological, economic, and social. A rural patient managing three chronic conditions may have a sound care plan on paper and still deteriorate at home — because no one helped set up the monitor, the nearest clinic is an hour away with no transportation, the broadband can't support a video visit, the medication is unaffordable, and there's no local navigator to hold it all together.
The result is missed appointments, poor monitoring between visits, medication nonadherence, preventable emergency-room visits, and avoidable hospital readmissions.
Many rural patients don't fail their care plans because they don't care about their health. They fail because the system is hard to navigate and the resources to comply are out of reach. The problem is rarely the patient — it's the missing infrastructure around them.
How we work
Devices and dashboards alone don't improve rural health. Someone has to reach the patient, set up the technology, build trust, follow up when readings stop, and clear the barriers in the way.
We hire and train Community Health Workers from the communities we serve, then transport them into patients' homes.
We deliver, install, and support remote monitoring devices, and confirm the first successful reading before we leave.
We screen for transportation, food, and medication barriers and connect patients to the resources that remove them.
Abnormal readings and concerns are escalated to licensed clinical partners under written protocols. We support clinicians — we never replace them.
